By scheduling an appointment or receiving services at Opalite Aesthetics, you acknowledge and agree to the following terms: Medical Information Clients are responsible for providing accurate and up-to-date information regarding their medical history, medications, allergies, health conditions, and pregnancy status. Withholding or failing to disclose relevant information may impact treatment safety and outcomes. Treatment Outcomes Aesthetic treatments are individualized, and results will vary from person to person. While our providers strive to deliver the highest standard of care, specific results cannot be guaranteed. Treatment success depends on a variety of factors, including individual response and adherence to provider recommendations. Consent to Treatment Prior to receiving treatment, clients must review and complete all required consent forms. By proceeding with services, you acknowledge that the procedure, potential risks, expected benefits, and available alternatives have been explained to you and that all questions have been addressed. Treatment Instructions Clients agree to follow all pre-treatment and post-treatment guidance provided by their provider. Opalite Aesthetics is not responsible for complications, adverse outcomes, or diminished results resulting from failure to follow recommended instructions. Assumption of Risk All medical and aesthetic procedures carry inherent risks. By choosing to proceed with treatment, clients acknowledge and accept these risks and release Opalite Aesthetics and its providers from liability associated with known treatment risks when services are performed according to accepted standards of care. Refund Policy All services, memberships, packages, and product purchases are final sale. Refunds are not available for completed treatments or opened products. Appointments and Cancellations Appointment times are reserved exclusively for each client. Cancellations, reschedules, late arrivals, and missed appointments may be subject to fees in accordance with clinic policies. Provider Discretion Opalite Aesthetics reserves the right to decline, postpone, or discontinue treatment when it is determined that a service may not be appropriate, safe, or in the client's best interest. Privacy and Confidentiality Protecting your personal information is important to us. All client records and health information are maintained in accordance with applicable privacy regulations, including HIPAA, and are handled with strict confidentiality.
